PDF Registration for October 2023 BOM
In order to use the pdf registration form, you must first download it to your computer and then open it with Adobe Reader. Most browser based pdf readers and other pdf utilities, particularly the default pdf program provided on Mac computers, will not correctly handle fillable form pdf files.
To download the file, right-click this link, then select "Save link as" to save the form on your computer. Then open it with Adobe Reader to fill out.
